17-12-2015 | Epilepsy Transition Nurse Co-ordinator (Adolescent & Transition Care (CNM3 Grade)

The role of the Community Epilepsy Transition Nurse will be a dynamic and developing role where the main aim is to improve the preparation and engagement for the transition of all young people with Epilepsy from acute paediatric service into adult services. This will include developing a standardized framework for the management of young people to adult Epilepsy services.

The post will be shared across the three paediatric hospitals working in partnership with Epilepsy Ireland. The post-holder will need to be proactive in pioneering and developing this role in response to the challenges around the on-going healthcare needs of young people and their parents/carers as well as developing independence towards managing health and their move into adult Epilepsy services. The post is intended to improve the outcomes as well as engage this age group in their care and future healthcare needs and follow up.

The successful candidate will have excellent clinical, evidenced based knowledge and skills and will demonstrate strong leadership skills. We offer professional development and support, allowing you the opportunity to expand your knowledge and skills in the above role.

It is envisaged that the post would be developed to Advanced Nurse Practitioner level.

 2. Five years post registration experience, two of which should be in the area of Epilepsy.

 3. BSc in Nursing or a Higher Diploma in Clinical Practice.

 4. Desirable – post graduate Diploma in Neurosciences or equivalent
